Quick Answer

Hernia recurrence after laparoscopic mesh repair is 1-2%. After open repair it is 2-5%. Main causes are wound infection, heavy lifting too soon, obesity, and chronic cough. Recurrent hernias can be re-operated successfully with laparoscopic technique.

Hernia Recurrence โ€” Why It Happens and How to Prevent It

A recurrent hernia โ€” when hernia comes back after surgery โ€” is one of the most common concerns patients have. Here is what the evidence says.

How Common Is Hernia Recurrence?

After laparoscopic mesh repair: 1 to 2% recurrence rate at 5 years. After open mesh repair (Lichtenstein): 2 to 5% recurrence. After open repair without mesh (old technique): 10 to 15% recurrence. The introduction of mesh repair has dramatically reduced recurrence rates from over 10% to under 5%.

Causes of Hernia Recurrence

Poor surgical technique: the most common cause. Surgeons with low hernia volumes have higher recurrence rates. This is why choosing an experienced laparoscopic hernia surgeon is critical.

Wound infection: infection compromises mesh integration and increases recurrence risk. Sterile technique and antibiotic prophylaxis minimise this.

Heavy lifting too soon: lifting heavy weights before the mesh has fully integrated (before 6 weeks) can dislodge the mesh.

Obesity: higher intra-abdominal pressure from obesity increases stress on the repair.

Chronic cough or constipation: persistent increased abdominal pressure. Treat the underlying cause before hernia surgery.

Smoking: impairs wound healing and increases infection risk. Stop smoking at least 4 weeks before surgery.

Treatment of Recurrent Hernia

Recurrent hernias can be successfully repaired. If the previous surgery was open, the recurrence is usually repaired laparoscopically (easier approach to the same area). If the previous surgery was laparoscopic, the recurrence may be repaired open or laparoscopically depending on the anatomy.

Cost of recurrent hernia surgery in India: Rs 70,000 to Rs 1,60,000. Slightly higher than primary repair due to adhesions and technical complexity.
